[ARCHIVED] Pharmacy Benefits Effective August 1, 2015

As a reminder, your NEW pharmacy benefits with OptumRx will begin on Saturday, August 1, 2015.

If you have not yet received your new prescription drug card and need to have a prescription filled (on or after Saturday, August 1st) you can provide the information below to your pharmacist.

Your Member ID is the same Member ID that is on your Anthem BCBS medical card.

To submit the claim to OptumRx, your pharmacy will need to submit the following information:

  • Rx Bin: 610494
  • Rx PCN: 9999
  • Rx GRP: COHRX
  • Member ID - obtained by contacting OptumRx.
    • For Members: 1-888-543-1369
    • For Pharmacists: 1-800-788-7871
  • Member Name

We have several cards that were returned to our office due to incorrect address.  We are working to get those out to the affected members today through interoffice mail.
