You may complete the Financial Hardship Certification Form and submit it, along with the required documentation, to the Treasurer’s Office for review. As of July 26, 2018, all inquiries regarding hardship/payment arrangements for EMS Cost Recovery Program bills are being handled by the Treasurer’s Office.
